We treat all of our service users as individuals with their own special needs. At the initial visit and before any service commences one of our senior supervisors visit to carry out needs as well as risk assessment.
With the participation of users, their families and representatives we draw up a Support or Service Plan, which include details of what tasks are to be performed, and a copy of this plan is given to the Service User. It is during this visit that our work is explained and a folder containing user guide, complaints system and our out-of-hours emergency contacts is left at the users’ home.
We involve relatives and representatives from the beginning so our services can be explained to them and their views are taken into account. We therefore welcome relative’s presence at this early planning stage and to be fully involved in drawing up the Service Plan.
Re-assessments and reviews:
All Support or Service or Care Plans are reviewed on a regular basis at intervals which are agreed when the Plan is drawn up. However, if circumstances change for better or worse a review can be arranged at very short notice. Families, representatives and Local Authorities are always invited to reviews and re-assessments.
